PARTICIPANT WAIVER AND RELEASE OF LIABILITY

In consideration of being allowed to participate in any Soxcessful LLC event(s) or activities, the undersigned acknowledges, appreciates, and agrees that:

Risk Acknowledgment: I understand and accept that participating in a Soxcessful LLC event(s) or activities involves inherent risks, including, but not limited to, physical injury, illness, or damage to personal property. I voluntarily assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation.

Medical Treatment: I agree that it is my responsibility to consult a physician before participating in any Soxcessful LLC event(s) or activities. The undersigned declares that binary/he/she is in good health and that no disability, impairment, injury, disease, or ailment prevents him from participating in active exercise, strenuous physical activity, or moving (walking/running);

In the event of injury, accident, and/or illness during this event(s) or activity, I consent to receiving medical treatment as deemed necessary. The cost of such medical treatment, including transportation, is solely my responsibility.

Photo/Video Release: I grant permission to Soxcessful LLC, its representatives, partners and employees to capture/take photographs, motion pictures, recordings, comments, feedback or videos; including but not limited to any liability arising by virtue of any blurring, distortion, alteration, illusion, editing, or use in composite form, whether intentional or otherwise, that may occur in the making or processing of the finished product. of me during event(s) or activities. I authorize the use of such photographs, motion pictures, recordings, comments, feedback or videos for any lawful purpose, including, but not limited to, promotional materials, record keeping, social media pages, website, public relations efforts without remuneration.

I consent to the use of my name, image, likeness and voice in any photographs, motion pictures, recordings, or video content of my participation, and I release Soxcessful LLC, its officers, directors, shareholders, employees, other participants, attorneys, agents, instructors, sponsors and advertisers, and all of their respective representatives and successors in interest (collectively, the “Releases”) from any and all liabilities, claims, debts, demands, suits, actions, causes of action, or rights of action, which I or my heirs, assigns, legal or personal representatives, or estate can, shall, or may ever have, now or in the future arising out of the production, distribution, broadcast or exhibition of any part and reproductions thereof in any medium now known or hereafter developed.

Personal Belongings: I acknowledge that I am responsible for the security and safety of my personal belongings during event(s) or activities. Soxcessful LLC is not responsible for any loss, theft, or damage to personal property.

Code of Conduct: I agree to abide by the rules and regulations set forth by Soxcessful LLC and to conduct myself in a manner that is respectful and considerate of others. Failure to do so may result in my removal from the event(s) or activities without refund if applicable.

Release and Waiver: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Soxcessful LLC, its officers, directors, shareholders, employees, other participants, attorneys, agents, instructors, sponsors and advertisers, and all of their respective representatives and successors in interest (collectively, the “Releases”) from any and all liabilities, claims, debts, demands, suits, actions, causes of action, or rights of action, which I or my heirs, assigns, legal or personal representatives, or estate can, shall, or may ever have, now or in the future, for, by reason of, or based upon any injury, death, illness, disease, or damage to or loss or theft of property arising out of, directly or indirectly, or in any way connected with or related to my participation in the event(s) or activities and/or use of, or entry into, the Facilities, and/or use of the Site, whether or not caused by any negligent acts or omissions of any releasee; or otherwise, to the fullest extent permitted by law.

Assumption of Risk: I understand that the event(s) or activities involve physical activity that may be strenuous and that I am responsible for determining whether I am physically fit and adequately skilled to participate. I assume all risks associated with my participation, including, but not limited to, falls, contact with other participants, weather conditions, and all other risks, known and unknown.

Indemnification: I agree to indemnify, defend, and hold harmless Soxcessful LLC, its officers, directors, shareholders, employees, other participants, attorneys, agents, instructors, sponsors and advertisers, and all of their respective representatives and successors in interest (collectively, the “Releases”) from any and all claims, demands, actions, causes of action, losses, liabilities, damages, and costs, including attorney's fees, arising out of or related to my participation in this event(s) or activities.

Severability: I agree that if any portion of this waiver is held to be invalid, the remaining portions shall continue in full force and effect.

Governing Law: This waiver shall be governed by and construed in accordance with the laws of the state in which the event(s) or activities is conducted.

By signing this waiver, you agree to the terms and conditions set forth bySoxcessful LLC, acknowledging the risks involved and releasing the organization from any liability related to your participation in the event/activity.
